Find a Provider
This is where you can find the name or location of a provider or hospital.
Instructions:
Search by last name. Fill in the last name of the provider or hospital you
are looking for.
Or…
Search by location. Fill in a ZIP code. To search in a general area, members
can enter the ZIP code and select how many miles around the ZIP code you
would like to search. You can also search by city.
Or …
Search by specialty. To find a provider by specialty, you can use the drop
down box.
Please remember:
1. Members need a referral from their Primary Care Physician (PCP) to visit
most specialists. Please call Member Services at 1-866-543-2167 if you need
more information on referrals.
2. Members need to call Member Services at 1-866-543-2167 before seeing a
new PCP.
