- Perform comprehensive assessments to understand your complete health care needs
- Asset with dental needs and referrals to community resources
- Assist with equipment needs such as glucometers, nebulizers, oxygen, pulse oximeters
and peak flow meters
- Provide education on the specific disease process, medications and need for tests,
procedures and necessary lab work
- Coordinate care with your primary care provider (PCP) and specialists
