Specialty Pharmacy Implementation
DPCI has entered into an agreement with Curascript Pharmacy to provide specialty
pharmacy products to provider offices for our members. Effective October 1, 2007,
a new pharmacy authorization form will need to be utilized for any of the injectables
and associated infusions that are provided in a physician’s office. This process
is related to injectable drugs and infusions (J-codes, Q-codes) that generally
are subject to prior authorization. If you have billed DPCI in the past for one
of the J-codes or Q-codes that require prior authorization, you should have received
a letter detailing this new process. If you bill J-codes and Q-codes to DPCI
and have not received a letter, please contact Taunja Durham (302) 894-6723 or

Self administered injectables are available through the Delaware Medicaid pharmacy
benefit administered by EDS. Members should be directed to Delaware retail pharmacies
with prescriptions to obtain these drugs. If there are clinical exceptions that
require the injectable drug to be given in the office setting, please submit the
clinical documentation to support the office administration with this request.
